Terms of Service

IMPORTANT NOTICE: THESE TERMS OF SERVICE CONTAIN A BINDING ARBITRATION PROVISION AND WAIVER OF CLASS ACTION RIGHTS AS DETAILED IN SECTION 12 (DISPUTE RESOLUTION & ARBITRATION) BELOW. PLEASE READ CAREFULLY.

YOU ACKNOWLEDGE AND AGREE THAT STATEWIDE BENEFITS LLC, OPERATING AS THE GOOD BENEFITS COMPANY, IS NOT AN INSURANCE COMPANY, INSURANCE PROVIDER, BROKER, OR AGENT. OUR MEMBERSHIP PROGRAM PROVIDES ACCESS TO BENEFITS, INCLUDING TERM LIFE INSURANCE, PERSONAL PROPERTY INSURANCE, AND OTHER SERVICES THROUGH THIRD-PARTY PROVIDERS.

These Terms of Service (“Terms”), along with our Privacy Policy and any supplemental terms applicable to specific membership plans or trial periods (collectively, the “Agreement”), form a legally binding agreement between you and Statewide Benefits LLC, operating as The Good Benefits Company® (“Good Benefits,” “we,” “our,” or “us”). Please read these Terms, our Privacy Policy, and any applicable supplemental terms carefully, as they govern your access to our website (https://www.goodbenefitsco.com/), enrollment in our membership program, and use of the benefits offered through the program (collectively, our “Services”).

You agree to be bound by these Terms by: (1) enrolling in our membership program; (2) accessing or using our website or Services; or (3) clicking “I Agree” or otherwise electronically accepting these Terms. We recommend saving or printing a copy of these Terms for your records.

1. About These Terms

1.1 Scope
These Terms govern: (a) your enrollment in and use of our membership program, which provides access to benefits such as personal property insurance, guaranteed issue life insurance, telemedicine, and other perks (collectively, “Membership Benefits”); (b) your access to and use of our website at https://www.goodbenefitsco.com/ (“Website”); and (c) your communications or interactions with Good Benefits.

1.2 Revisions to Terms
We may revise these Terms at our sole discretion. Changes are effective immediately upon posting on our Website and apply to all subsequent access and use of our Services. Changes to the dispute resolution provisions in Section 12 will not apply to disputes for which we have actual notice before the change is posted. Your continued use of our Services after changes are posted constitutes acceptance of the revised Terms. Please check this page regularly for updates.

1.3 Privacy Policy
Our Privacy Policy, available at https://www.goodbenefitsco.com/privacy-policy, is incorporated into this Agreement. It describes how we collect, process, and use your personal information in connection with our Services. You acknowledge that we may use your personal information as described in the Privacy Policy.

2. Use of Our Services

2.1 Description of Services
Our membership program offers access to Membership Benefits, provided through third-party providers, including but not limited to:
Personal Property Insurance (PPI): Covers personal property items (e.g., washer, dryer, refrigerator, TV, computer, cell phone, clothing, furniture, eyeglasses, hearing aids, beds, artwork, appliances) up to $2,000 per claim, with unlimited claims per household and a $100 deductible per claim. Covers damage or loss due to fire, flood, earthquake, tornado, hurricane, or theft.
Guaranteed Issue Life Insurance: Up to $10,000 of life insurance with no medical exams or invasive procedures.
Telemedicine: Physical and mental health services delivered by third-party provider Recuro Health.
UPS Discount: 10%* on UPS Next Day Air® Early
50% on Domestic Next Day / Deferred
30% on Ground Commercial / Residential
International: 50% on Export / 40% on Import / 25% Canada StandardSavings begin at 75%* on UPS Freight® shipments over 150 lbs.
In addition, members can take advantage of UPS Smart Pickup® service for free
Aetna Dental Discount: 15% to 50% off dental procedures at participating dentists.
Roadside Assistance: Roadside assistance calls for jump-starts, 3 gallons of gas delivery, unlocking vehicles, flat tire repair, or towing to the nearest authorized repair facility.  Coverage is $50 per call in one continuous 7-day period.
Discounted Pharmacy: 70-80% off medications to bridge high-deductible plans or cover non-insured medications.
Mobile Device Protection: Covers up to four mobile devices per family with a $25 deductible for repair or replacement. Details and master policies for these benefits are available in the member area of our Website. We reserve the right to modify or discontinue any Membership Benefits at our discretion. By using our Services, you agree to us processing your personal information to provide these benefits, in accordance with our Privacy Policy.

2.2 Eligibility
To use our Services, you must: (a) be at least 18 years old; (b) have a valid payment method; (c) be legally authorized to enter into this Agreement; (d) comply with all applicable laws; and (e) be in good standing with Good Benefits. Our Services are available only in the following states and district: Alabama, Arizona, Arkansas, California, Connecticut, Delaware, Florida, Georgia, Hawaii, Idaho, Illinois, Indiana, Iowa, Kansas, Kentucky, Louisiana, Michigan, Mississippi, Nebraska, New Jersey, North Dakota, Ohio, Oklahoma, Rhode Island, South Carolina, Tennessee, Texas, Virginia, Washington DC, West Virginia, Wisconsin, and Wyoming. By using our Services, you represent that you meet these requirements and have the authority to enter into this Agreement.

2.3 Access to Services
You are responsible for arranging access to our Services (e.g., internet connection) and ensuring that anyone accessing our Services through your connection complies with these Terms. We are not liable if our Services are unavailable for any reason. We may restrict access to certain parts of our Services at our discretion.

2.4 Third-Party Links
Our Website may include links to third-party websites or services not controlled by Good Benefits. We do not endorse or assume responsibility for these third-party sites. Your use of such sites is at your own risk, and these Terms and our Privacy Policy do not apply to them.

2.5 License to Use Services
Subject to this Agreement, we grant you a non-exclusive, non-transferable, revocable license to use our Services. We reserve all rights not expressly granted. We may terminate this license at any time for any reason.

2.6 General Representations
You represent that your use of our Services will: (a) comply with this Agreement and applicable laws; (b) not impersonate others or harm Good Benefits or other users; (c) not interfere with the functionality of our Services; (d) not infringe our intellectual property, including our trademark “The Good Benefits Company®”; and (e) not involve unauthorized use of our data or systems.

2.7 Rights Granted to Good Benefits
By using our Services, you grant us a worldwide, royalty-free, non-exclusive license to use any information you provide (“Content”) to deliver our Services, as described in these Terms and our Privacy Policy. You represent that you have the right to provide such Content.

3. Membership Enrollment

3.1 Creating an Account
To access our Services, you must create a membership account (“Account”) by providing accurate personal information and a valid payment method. You may need to complete identity verification or security measures to access your Account or specific benefits.

3.2 Account Security
You are responsible for maintaining the confidentiality of your Account credentials (e.g., username, password). Do not share your credentials with others. Notify us immediately of any unauthorized access or security breach at member@goodbenefitsco.com or +1 888 606 4252. We may disable your Account if we suspect a violation of these Terms or a security issue.

3.3 Membership Subscriptions
Enrollment requires providing accurate personal information. You represent that all information provided is complete and accurate. Misrepresenting your authority or obtaining benefits for unauthorized individuals may violate applicable laws.

4. Membership Benefits

4.1 General Disclaimer
Good Benefits is not an insurance company, broker, or agent. We facilitate access to Membership Benefits through third-party licensed insurance carriers and service providers. We do not underwrite insurance policies or guarantee the availability, terms, or outcomes of these benefits, which are subject to third-party provider policies. Details and master policies are available in the member area of our Website.

4.2 Third-Party Providers
All Membership Benefits are provided by third-party providers. We are not responsible for how these providers handle your personal information or deliver their services. Additional terms may apply when you enroll in third-party benefits, and you should review those terms carefully. Your use of third-party benefits is at your own risk.

4.3 Authorization to Share Information

By enrolling in our membership program, you authorize us to collect and share your personal information with third-party providers as necessary to deliver Membership Benefits. You also authorize these providers to share relevant information (e.g., policy details) with us to manage your membership.

5. Fees and Payment

5.1 Membership FeesEnrollment in our membership program requires a recurring subscription fee of $34.99 per month, billed monthly from the date of enrollment. Fees are subject to change with notice, as permitted by law. Fees are charged to your chosen payment method at the start of each billing cycle.

5.2 Payment Information
You must provide accurate and current payment information. You are responsible for all charges incurred and any applicable taxes. If your payment method changes, update it through your Account or contact customer service at member@goodbenefitsco.com or +1 888 606 4252.

5.3 Automatic Renewal
Subscriptions renew automatically unless canceled before the billing cycle ends. You authorize us to charge the applicable fee and taxes to your payment method on file. Manage or cancel your subscription through your Account or by contacting customer service.

5.4 Cancellation
You may cancel your membership at any time by contacting customer service at member@goodbenefitsco.com or +1 888 606 4252, or using the cancellation option in your Account (where available). Upon cancellation, access to Membership Benefits will cease. No refunds are provided for unused portions of a subscription.

6. Communications

6.1 Good Benefits Communications
We may contact you via email, phone, or SMS to provide updates about your membership or benefits. By providing your contact information, including your phone number (+1 888 606 4252 for SMS), you consent to receive communications, including automated calls or texts, for service-related and marketing purposes. You may opt out of non-essential communications by replying “STOP” to SMS messages, adjusting your Account settings, or contacting customer service. Message and data rates may apply.

6.2 Call Recordings
We may record phone calls for quality assurance and fraud prevention. By contacting us at +1 888 606 4252, you consent to such recordings.

6.3 Communication Delays
We are not liable for delays, failures, or errors in delivering communications due to factors beyond our control (e.g., network issues). You are responsible for maintaining accurate contact information in your Account.

7. Intellectual Property

7.1 Ownership
Good Benefits retains all rights to our Website, Services, and related content, including our registered trademark “The Good Benefits Company®” and all copyrights. Using our Services does not grant you any rights to use our intellectual property without permission. Third-party trademarks used in connection with our Services remain the property of their respective owners.

7.2 Feedback
If you provide feedback or suggestions about our Services, you assign all intellectual property rights in such feedback to Good Benefits. We may use feedback without obligation to you.

8. SMS Short Code Terms
When you opt-in to our SMS service at +1 888 606 4252, we will send a confirmation message. This service provides alerts about your membership, such as benefit updates. To cancel, text “STOP” to +1 888 606 4252; you will receive a confirmation of unsubscription. To rejoin, sign up again. For help, text “HELP” to receive instructions. Message and data rates may apply. We are not liable for delayed or undelivered messages. Contact member@goodbenefitsco.com for questions.

9. Disclaimers
Our Services are provided “as is” without warranties, express or implied, including warranties of merchantability, fitness for a particular purpose, or non-infringement. We do not guarantee that our Services will be uninterrupted, error-free, or meet your expectations. We are not liable for issues arising from third-party providers, data transmission failures, or inaccuracies in benefit delivery.

10. Limitation of Liability
Statewide Benefits LLC, its affiliates, and subsidiaries will not be liable for indirect, incidental, punitive, or consequential damages (e.g., lost profits, data loss) arising from your use of our Services, regardless of the legal theory. Our total liability for any claims under this Agreement will not exceed $100. If applicable law limits this provision, our liability will be limited to the maximum extent permissible.

11. Indemnity
You agree to indemnify and hold harmless Statewide Benefits LLC, its affiliates, and their employees, contractors, and agents from any claims, damages, or expenses (including attorney’s fees) arising from: (a) your use of our Services; (b) your violation of this Agreement; (c) your violation of third-party rights; or (d) unauthorized access to your Account.

12. Dispute Resolution & Arbitration

12.1 Governing LawThis Agreement is governed by the laws of the State of Delaware, excluding conflicts of law provisions. Arbitration procedures are governed by the Federal Arbitration Act.

12.2 Initial Dispute Resolution
Before pursuing formal legal action, you must notify us in writing of the dispute, referencing the relevant Terms. Within 30 days, we will attempt to resolve the dispute in good faith. 12.3 Binding Arbitration
If the dispute is not resolved within 30 days, either party may initiate binding arbitration through JAMS, following JAMS rules (available at www.jamsadr.com). Arbitration is the sole means to resolve claims, except for injunctive relief or small claims court actions. The arbitrator’s decision is final and binding. Payment of filing and arbitrator fees follows JAMS rules, with Good Benefits reimbursing your initial filing fee if it exceeds court filing fees in your county or Delaware.

12.4 Class Action Waiver
Arbitration will be conducted individually, not as a class action. You waive any right to participate in a class action against Good Benefits. If a court finds this waiver unenforceable for a specific remedy, that remedy will be severed and pursued in court, with other claims arbitrated.

12.5 Opt-Out
You may opt out of arbitration by sending written notice to the address above within 30 days of accepting these Terms. If you opt out, Good Benefits is also not bound by arbitration.

12.6 Exception
Either party may pursue intellectual property claims or small claims court actions in a court of competent jurisdiction. Intellectual property claims include protection of our trademark “The Good Benefits Company®” but exclude feedback ownership disputes, which are arbitrated.

12.7 Changes
We will provide 30 days’ notice of changes to this section. Continued use after the 30th day subjects unfiled claims to the revised terms.

12.8 Non-Arbitrated Claims
For disputes not subject to arbitration, you and Good Benefits submit to the exclusive jurisdiction of federal and state courts in Delaware. You waive jurisdictional and venue defenses.

13. Miscellaneous

13.1 Entire Agreement
This Agreement is the complete understanding between you and Good Benefits, superseding all prior agreements.

13.2 Severability
If any provision of these Terms is invalid, the remaining provisions remain in effect.

13.3 Assignment
You may not assign your rights under this Agreement without our consent. This Agreement binds the parties’ successors and assigns.

13.4 Force Majeure
We are not liable for delays or failures due to events beyond our control (e.g., natural disasters, network failures).

13.5 United Family Association Terms of Membership
United Family Association, from now on known as “Association”. Please read the below membership and billing agreement. Signing below will indicate that you agree to the terms of membership and billing. Member is an individual who resides in the United States, 18 years of age or older and otherwise eligible to receive services. This definition also includes the Member’s dependents: legal spouse and/ or children under the age of 26 years residing with the Member. A civil union spouse is considered a legal spouse for policies issued in the state of Illinois. I am requesting enrollment as a Member of the Association and understand that there are dues associated with the membership. I understand that my failure to remit membership dues will result in loss of eligibility to participate in any of the Association sponsored programs or discounts. I understand this is not an insurance program or product. Applications for membership received between the 1st and 28th of the month where the payment was successfully processed will be considered in good standing as of the 1st of the month. Applications for membership received between the 28th and the end of the month where the payment was successfully processed will be considered in good standing on the 1st of the following month. Member understands that the Association will make every reasonable effort to ensure the information provided on its website is accurate at the time of initial posting; however, Member understands that it is the MEMBER’S RESPONSIBILITY to confirm the information listed with the product or service provider is still accurate and valid as of the date of purchase from the service provider. Member will contact Association in the event any benefit, discount or offer listed on the Association website purported by Member to be inaccurate in any way. Member understands that the Association does not set the discounts or services offered by the individual service provider; therefore, the Association cannot guarantee that any specific savings will be realized. It is the responsibility of the Member to verify service provider charges and make an informed determination of potential savings based upon market comparison. Member further understands that the Association and its affiliates, directors, officers, employees and representatives are not responsible for the outcome of any services received from any service provider associated with the Association either directly or indirectly; and, Member agrees to hold the Association and its affiliates, directors, officers, employees, representatives and associates harmless for any undesirable outcome or damage or loss caused by or resulting from any service, product or action of a service provider. You further affirm you are not enrolling for someone else, another entity - or on your own behalf, for the purpose of, in whole or in part, of making threats or demands for money by alleging violations of the Telephone Consumer Protection Act and/or Federal Trade Commission DO NOT CALL registry or regulations. You expressly indemnify all parties involved in enrolling in these product(s) from any alleged violations, or threats of litigation, as it relates to the Telephone Consumer Protection Act or Federal Trace Commission DO NOT CALL registry related violations.
